Terms of this contract between Signature Limousine Services and company named above
Customer will be issued an Account Number that must be given at the time of ordering a limousine. If no account number was specified,
a payment will be expected from the customer that uses the service. Billing will be set up for every week, 15 days or monthly by Signature
Limousine Services upon approval of this application. The client will receive, via e-mail or fax, the invoice and will need to be approved for
payment by sending via e-mail authorization for payout on the invoice. Payment is due upon receipt of the invoice. If payment is not received
by the due date, a 5% late fee will be added to your account for next billing period. If payment of the full amount due is not received during
the second billing period, the full amount will be charged to your account.
Signature Limousine Services agrees to provide prompt service for the agreed upon price, based on customer’s specifications regarding
pick up time, pick up address and destination. A grace wait time for airport domestic arrival is 30 minutes after landing and 45 minutes after
landing at the international airport. There is a 15-minute grace period for regular appointment pickups ( no airport ). After the grace period has
elapsed, there is a charge of $15.00 for every 15 minutes on sedans, $20 for luxury sedans and vans and $50 for stretch limos.

For ANY CANCELLATION of any reservations, we need a minimum of 1 hour advanced notice; otherwise the account will be charged for
the full amount of the trip.
A 20% automatic gratuity is added to each charge.
A premium rate of $10.00 applies to the pick-ups between 11:00 pm - 5:00 am.
Any “No Show” (i.e. reservation not cancelled properly) will be charged to the account with 20% gratuity.
In complete agreement and accord, I accept this agreement personally and as an officer of the company named above.
I guarantee as an officer of the company named above all invoices tendered under this agreement.
